Cizek, 79, Michigan City, will be held at 1:30 p.m. Monday at Miller Voorhis Funeral Home. He died at 6:45 p.m. Thursday at the Michigan City Health Care Center. Born Oct. 22, 1910, in Chicago, he was the son of Vaclav and Marie Cizek. He was married May 12,1934, to Mildred M. Wilson, who died Sept. 12, 1981. He was the manager of the Sears retail store in Monticello from 1952 until he reared in 1972. He had lived in Michigan City since 1976. Prior to that he had lived in Lafayette for two years. He was a member of the Monticello First Presbyterian Church. He was a past master of the Rensselaer Masonic Lodge. In Michigan City, he was a member of the Masonic Lodge, Order of the Eastern Star, Orak Shrine and Royal Arch Mason Commandery. He is survived by his daughter, Sherry Altman, Michigan City. Three brothers and one sister preceded him in death. The Rev. Edward Morris will officiate at the service. Burial will be in Riverview Cemetery. Friends may call after noon Monday at the funeral home. Memorial contributions may be made to the Indiana Masonic Home or the American Cancer Society.
