On this 4th day of June, A.D. one thousand nine hundred and twelve, personally appeared before me, a Notary Public within and for the county and State aforesaid, William T. Laddusaw, who, being duly sworn according to law, declares that he is 73 years of age, and a resident of Brookings, county of Brookings, State of South Dakota; and that he is the identical person who was enrolled at Decatur County, Iowa under the name of William T. Laddusaw on the 15th day of August 1862, as a private in Co. I- 34th Iowa in the service of the United States, in the Civil war, and was Honorably Discharged at Washington D.C., on the 28th day of October 1865. That he also served Honor Guards of Iowa state Militia State of Missouri. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height 5 feet 10 inches; complexion, Fair; color of eyes, Hazel; color of hair, Brown; that his occupation was farmer, that he was born June 20th, 1838, at Dearborn County, Indiana. That his several places of residence since leaving the service have been as follows: Decatur County Iowa from Nov 1866 to 6/20-69 Plymouth County Iowa from 7/20/1869 to 2/1-1892 Brookings County SD from 3/1- 1892 to present. That he is a pensioner under certificate No. 365702. That he has not applied for pension under original No. 365702. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912. That his post-office address is Brookings, county of Brookings, state of South Dakota. Attest John Alton, H. C.
