Interment in Bethany Cemetery. MRS. AMANDA FORMAN DIES AFTER A BRIEF ILLNESS Mrs. Amanda Ellen Forman, widow of Samuel T. Forman, died last Wednesday morning at 5 o'clock at her home northeast of Shelbyville, following a short illness,death being due to heart trouble. She had been in poor health for some time and recently suffered an attack of flu, but was apparently recovering and seemed better than usual on Tuesday evening. Early Wednesday morning she was seized with a coughing spell and passed away a few hours later. Funeral services were held Friday morning at 11 o'clock at Bethany Church, conducted by Rev. S. E. Hoover. Interment was in the church cemetery. Mrs. Forman, formerly Amanda Ellen Baker, was born on September 27, 1847, in Kentucky. She came with her parents to Shelby county when four years old and had lived here seventy years. She was married in 1865 to Samuel T. Forman, who died in 1913. There were thirteen children, eight of whom survive: Arthur M. Forman, of Florence, Ariz.; Joe, Ellis and Harry H., Mrs. George Duncan, Mrs. P. M. Perrigo, Mrs. Wm. Vanzant and Mrs. J. O. Glahn, all of this county. Four brothers, George, Neal, Marsh and Richard Baker, all of this county, also survive.
