In the 1900 Census he was residing at 315 5th St., Ottumwa (Ward 2), Center Twp., Wapello Co., IA, with his wife, a daughter, Dorothy B., aged 4, and a son, Dwight F, 2, both born in IA, and his widowed mother. Frederick was a dentist. In 1910 the family was at the same address. By this time, Frederick and Gertrude had been married 15 years, and had had two children, both living. Frederick, aged 42, died on 5 Jan 1914 at Spirit Lake, Dickinson Co., IA. His obituary was published in the Dentists Record, Volume 10, pp. 428-429, edited by Frank F. Worthan: FREDERICK ARTHUR LEWIS, D.D.S DIED AT SPIRIT LAKE, IOWA, JAN. 3, 1914 The subject of this sketch was born in Ottumwa, Iowa, Jun 9, 1871. His parents were Dr. Chas. G. Lewis and Anna Ball Lewis. He completed the course of study in the public schools of Ottumwa, graduating from the high school in 1891. He was a member of Company G., Iowa National Guards, for some time, being honorably discharge on account of physical disability. After graduating from the public schools, he attended S.U.I. dental department,taking his third year at University of Pennsylvania,where he received his degree of D.D.S. in 1894. He entered practice in Ottumwa, Iowa, continuing there till 1910, when he moved to Spirit Lake, Iowa, to benefit his health, having been a sufferer from asthma for many years. He was married to Gertrude Brown of Fairfield, Sept. 10, 1895. During his residence in Ottumwa he was active in the State Dental Society and was a member of the Board of Dental Examiners and took the initiative in the movement which resulted in a new dental law for Iowa and a reorganization of the State Board of Dental Examiners, of which he was secretary for several years. He is survived by three children, Dorothy B., Dwight F. and Chas. A., who are living in Spirit Lake, Iowa; also by one brother, Chas. B. Lewis, of Ottumwa, Iowa. NOTE: Frederick and Gertrude had a third child, Charles Arthur, born 12 Mar 1912 in Spirit Lake, about 2 years before Frederick died. He was still living at Spirit Lake in the 1915 Iowa State Census. In 1920, he and his mother were boarding with widow Smith and her daughter in Cedar Rapids, IA. In 1930 Gertrude and Charles were in Roswell, NM, and according to the 1940 Census, were living in Roswell in 1935. By 1940, Charles had married Norma ______ and they were living near Bakersfield, Kern Co, CA. He was an auto mechanic and Norma was a newspaper proofreader. Charles, residing in New Hall, CA, received a certificate admitting him to Brazil, and the certification indicates permanently. He has not been found since.
