Since that time, her health has been gradually failing, Mrs. Dvorak was born May 19, 1878 in Tama county, the daughter of Joe and Mary Formanek. She was one of a family of 12 children and her death is the first in this family of children. She was united in marriage to Joe Dvorak November 18, 1896, at Toledo. They became the parents of six children Libby and Milo are deceased as well as the husband who died in August, 1931. Surviving are the two daughters. Mrs. Ed Plehal and Mrs. Bernard Glovka, both of Chelsea . Also, two sons, Fred of Irving and Joe of Belle Plaine; three grandchildren, Betty Jean and Norman Dvorak and Jeanette; 11 brothers and sisters Mrs. W. T. Clore, Lincoln, Nebraska; Mrs. John Wagner and Frank Formanek , Marengo; Amos Formanek, Ladora; Joe Formanek, Toledo; Mrs. Charles Kratoska, Ames; Mrs. Agnes Kratoska, and Mrs. Eleanor McGuire, Cedar Rapids; James Formanek, Vining: Charles Formanek and Mrs. Carl Benner of this city. Also, she has many nieces and nephews who survive. She was a member of the Z.C.B.J. lodge of Irving. Services were conducted by Theodore B. Hlubucek of Cedar Rapids. Mrs. Charles Upah and Mrs. Eldo Anders sang, accompanied by Mrs. Marie Christman as organist. Pallbearers were John Shanda, Emil Sikir, Frank M. Dvorak, Charles Weaver, William Vileta and Joseph Blaha. Interment was in the National cemetery at Vining. Many relatives and friends attended the services from Marysville, Kansas, Lincoln, Nebraska, Ames, Cedar Rapids, Marengo, Toledo, Ladora, Chelsea and other surrounding towns.
