Born on Oct. 12, 1903 in Maryland, she was the daughter of the late John and Juliana Lukac. In addition to her parents, she was preceded in death by her husband, Paul Sivak on May 22, 1976. Survivors include six children, Sister Mary Augustine of Hammond, Ind., Pauline "Cookie" Hoffman of Sterling, Va., John Sivak of Lexington Park, Albert Sivak of Locust Grove, Va., Paul Sivak of Colonial Beach, Va., and Francis “Mike” Sivak of Middletown, Ohio....
